Clinch River Health Services will be offering free athletic physicals for all Scott County prospective athletes, including rising 8th graders for the 2021-2022 school year. Physicals will be offered every Wednesday in July from 9 a.m. to 4 p.m. at Clinch River Health Services’ Medical facility located at 17285 Veterans Memorial Highway, Dungannon, Virginia. Appointments are required. Call (276) 467-2201 to make an appointment.
Clinch River Health Services is also requiring the completion of the CRHS Sports Physical Packet. Please bring the completed forms with you on the day of the physicals. Prior to the day of the physical, you will receive a conformation call with your arrival/appointment time. Please arrive at your arrival/appointment time with your CRHS Sports Physical Packet signed and completed or your appointment will be rescheduled.
We ask that all participants maintain social distancing standards.
DUE TO COVID-19, WE ARE REQUIRING:
- All participants to wear facemasks at all times while inside the facility.
- Only one parent/guardian per athlete/family allowed
- Athletes must fill out and bring VHSL forms that is included in the Clinch River Health Services Sports Physical Packet required forms.
*A sports physical does not replace a school physical or an annual visit to your primary care provider. Appointment is Necessary. A parent or guardian must accompany students under 18 years old. Students are asked not drink energy drinks for 24 hours prior to your physical. They cause the heart rate to rise to an abnormal rate. Please bring/wear your glasses or contacts for the eye exam
